Weeks 1–2
Pain
Turn the volume down.
Breathing, positioning and gentle hands-on work teach the pelvic floor and everything around it to stop holding on.
What We Treat
You have probably not said this out loud to anyone yet. It is treatable, it is private, and it starts with a cause-first evaluation and a conversation that moves at your pace.

Why It Happens
Pelvic floor muscles behave like any other muscles. They can be weak, or they can be too tight to let go, and the tight version is the one that surprises people. Pregnancy and delivery, surgery, an old back or hip problem, constipation, or years of quietly holding tension all change how those muscles work. Nerves in the area get sensitive when the pain has been around a while.
Weak and too tight can feel the same from the inside, and the wrong exercise makes one of them worse. So this starts with a cause-first evaluation: your history, your day, what makes it better and what makes it worse. Kegels found online are the usual first attempt, and for a tight pelvic floor they push in the wrong direction. Nothing happens here that you have not agreed to first.
The Honest Truth
Nobody regrets starting too early. Every week, someone in our clinic says they regret waiting.
How We Fix It
Every plan has an endpoint. Here’s what Pain, Prime, Perform and Prevent look like for pelvic pain.
Weeks 1–2
Pain
Breathing, positioning and gentle hands-on work teach the pelvic floor and everything around it to stop holding on.
Weeks 2–6
Prime
You learn to relax and to engage on purpose, in that order, because most pelvic floors have lost one of the two.
Weeks 6–10
Perform
Lifting, standing, walking and core work get added, so pressure has somewhere to go other than straight down.
Weeks 10+
Prevent
You leave knowing what your pelvic floor needs from you week to week: that’s the endpoint, not another appointment.

Questions, Answered
No. California lets you start with a licensed physical therapist directly, so nobody has to send you. Bring whatever notes or records you already have, and if you have none, that is fine too. If something in your history should be looked at by a physician, we will tell you and help you get there.
Mostly you talk, and somebody listens. It is one room, one door, a licensed physical therapist, and questions about your history and your day. Any hands-on assessment is explained before it happens, and it happens only with your permission. You can decline any part of it and still leave with a plan, and you are welcome to bring someone with you.
You get a plan with an endpoint, not an open tab of appointments. Pelvic floor work runs through the same four phases, Pain, Prime, Perform and Prevent, and the length depends on how long this has been going on. At your evaluation you will hear what your plan looks like and what has to be true to finish it. Progress gets measured in your own days, not in visits attended.
