What this policy covers.

This policy explains what this website collects, why, and what happens to it. It applies to Jewel Physical Therapy at 17750 Sherman Way, Suite 200, Reseda, CA 91335.

There are three forms on this site. Nothing on this site asks you for information it does not need, and none of the three requires you to create an account.

The booking form.

When you request a consult, the form collects your name, your phone number, your email address, and a short description of what is going on with you.

We use that to call or email you back and get you on the schedule, and to make sure the person who sees you already knows why you came in. That is the entire purpose.

The free benefits check.

If you ask us to check your coverage before your first visit, that form collects more, because insurers require more. It collects:

• Your first name and last name
• Your date of birth
• Your ZIP code
• Your insurance provider
• Your member ID
• Your group number, which is optional
• Your phone number and email address

That is health-plan information, and we treat it as confidential. It is used for one thing: verifying what your plan covers so we can tell you where you stand before you commit to anything. It is not used for marketing, and it is not passed around.

The 30-second pain check.

The pain check asks three questions: what area hurts, how long it has been going on, and how much it is affecting what you do. It submits those three answers only.

There is no name, no phone number and no email attached to it. We cannot tie a pain check back to a person, which means you can use it without identifying yourself at all.

Text messages are optional, and separate.

The booking form has a checkbox asking whether you are happy to receive text messages from us. It is optional. Leaving it unchecked does not affect your booking in any way, and you will still get a call back.

If you do opt in and later change your mind, reply STOP to any text and the messages end. You can also just tell us on the phone.

We do not sell your information.

Your information is not sold, rented or traded. It is used to contact you, to verify your coverage when you have asked us to, and to care for you as a patient. Nothing else.

Asking for a copy, or asking us to delete it.

You can ask us what we hold on you, ask for a copy of it, or ask us to delete it. Call (818) 341-3791 or email admin@jewelptinc.com and say which one you want.

We may need to confirm who you are first, so that nobody else can request your information. Where a law requires us to keep part of a medical record for a period of time, we will keep only that part and tell you plainly that we have.

What this site loads from other companies.

Two things, and we would rather list them than pretend there are none:

• Google Fonts serves the typefaces used on these pages, so your browser requests the font files from Google.
• Elfsight powers the widget that displays our public Google reviews, so that section is loaded from Elfsight.

Both are loaded so the page can display properly. Neither is given your form submissions.
