Weeks 1–2
Pain
Swelling down, quad switched on.
Swelling control, gentle range and quad activation come first, because a knee full of fluid will not bend for anybody.
What We Treat
A new knee does not bend on its own, and the weeks when that bend comes easiest do not last. It starts with a cause-first evaluation, then a plan matched to your surgeon’s protocol.

Why It Happens
A replacement fixes the joint surface. It does not hand back your motion, and motion is where this recovery is won or lost. Swelling limits how far the knee will travel. Scar tissue thickens in the range you are not using. The quad switches off after surgery and stays quiet longer than people expect. And the good leg takes over, which hides how little the new knee is doing.
The range you build early tends to be the range you keep, because that window narrows as scar tissue matures. So this starts with a cause-first evaluation: your bend and your straightening get measured, the swelling gets checked, somebody watches you walk, then the plan pushes where it counts and holds where it does not. Waiting until the knee feels ready usually means fighting later for motion that was easier to win now.
The Honest Truth
Nobody regrets starting too early. Every week, someone in our clinic says they regret waiting.
How We Fix It
Every plan has an endpoint. Here’s what Pain, Prime, Perform and Prevent look like after a knee replacement.
Weeks 1–2
Pain
Swelling control, gentle range and quad activation come first, because a knee full of fluid will not bend for anybody.
Weeks 2–6
Prime
Hands-on mobilization and measured range work chase full straightening and more bend, every single session.
Weeks 6–10
Perform
Progressive quad, glute and calf strengthening turns the range you own into steps you stop thinking about.
Weeks 10+
Prevent
Stairs, uneven ground and distance get tested before you are cleared, and you keep the routine that protects the knee: that’s the endpoint, not another appointment.
This is my first physical therapy experience, and I’m very pleased with the facility and staff. The therapist, Jewel, provided excellent and compassionate care. My initial session was highly beneficial and significantly alleviated my discomfort.

Questions, Answered
Early, and your surgeon sets the schedule. The first weeks are when range of motion comes easiest, so the sooner somebody is measuring it, the higher your ceiling tends to be. Bring your discharge paperwork and a licensed physical therapist will build the plan around it. In California you do not need a new referral to be seen.
Working near your limit is uncomfortable, and that discomfort is not damage to the implant. What matters is how the knee behaves afterwards: soreness that settles by the next session is normal, while swelling and pain that keep climbing mean the dose was too high. A licensed physical therapist adjusts that dose every visit, and you will always know why. Backing off completely is what costs you the bend.
You get a plan with an endpoint, not an open tab of appointments. Knee rehab runs through all four phases, Pain, Prime, Perform and Prevent, and progress gets tracked in degrees of bend and straightening rather than appointments attended. At your evaluation you will hear what your plan looks like and what has to be true to finish it. We also verify your insurance benefits before your first visit.
