Sciatica is not a diagnosis, it is a description. A nerve leaving your low back is being irritated, and the pain shows up along its route: the buttock, the back of the thigh, sometimes all the way to the foot. What irritates it is usually one of four things. A disc that has bulged and is pressing where the nerve exits. Spinal joints that stiffened up, so two or three segments carry the load meant for ten. Deep abdominal and hip muscles that quietly switched off after the last flare-up. And the way you sit, bend and lift all day, which keeps reloading the same spot before it can settle.

What people ask before booking for low back pain and sciatica.

In most cases, neither. California lets you see a licensed physical therapist without a physician referral, so there is nothing to schedule ahead of us. Most low back pain also improves without imaging, and MRIs regularly show disc bulges and wear in people who have no pain at all, so a scan does not always point at the cause. We start with a cause-first evaluation, and if imaging is genuinely necessary, we help you get it.

In most cases movement helps more than rest does, and hurt does not automatically mean harm. With sciatica the direction matters: some movements take pressure off the nerve and some add to it, and sorting out which is which is the whole point of the evaluation. Complete rest tends to leave a back stiffer and weaker, which sets up the next episode. We start with whatever calms your leg, one-on-one with a licensed physical therapist, and progress as the nerve quiets down.

You get a plan with an endpoint, not an open string of appointments. Your plan runs through the four phases, Pain, Prime, Perform and Prevent, and you will always know which phase you are in and what is left before the last visit. How long it takes depends on how long the leg pain has been there and what is irritating the nerve, so we give you the number after your evaluation instead of guessing before it.
